We are delighted to learn of your interest in Creighton University and to furnish you with the forms
for making application to the School of Dentistry. A special instruction sheet is on the back for your
convenience and guidance in completing your Supplementary Application form.
At this point, we remind you that the minimum requirement to receive consideration for admissions
is at least 2 (two) years of study - 64 semester hours. These 64 semester hours must be completed prior to
enrollment and must include the following: GENERAL BIOLOGY (6 SEMESTER HOURS);
GENERAL PHYSICS (6 SEMESTER HOURS); INORGANIC CHEMISTRY (8 SEMESTER
HOURS); ORGANIC CHEMISTRY (6 SEMESTER HOURS); AND ENGLISH (6 SEMESTER
HOURS). These courses must have been taken in an accredited College of Arts and Sciences.
You are required to take the Dental Admissions Test (DAT) and it is recommended that you take
this examination no later than July 2021. This would give you time for a retake (if one is needed) before
the first round of acceptances are sent out. You can register online at www.ada.org/dat.aspx Just follow
the links to find the DAT registration application.
Be certain to take care of all matters noted on the instruction sheet. Do so promptly.
Applications are received up to February 1, 2022. However, early application is strongly encouraged.
The Admissions Committee will begin reviewing completed applications August 1, 2021 and initial
acceptance will be tendered December 15, 2021. No consideration will be given to your application
until all the credentials requested have been received, INCLUDING DAT SCORES. All required
admissions credentials must be received in our Admissions Office by MARCH 1, 2022. An
incomplete application after this deadline will not be reviewed by the Committee on Admissions.
THE GREAT NUMBER OF APPLICATIONS TO BE CONSIDERED PROHIBITS THE
UNIVERSITY FROM ASSUMING RESPONSIBILITY IN WRITING TO OTHER SCHOOLS
FOR INFORMATION OR ADVISING YOU OF THE INCOMPLETE STATUS OF YOUR
APPLICATION. It is recommended that you monitor the status of your application at every dental
school to which you have applied through AADSAS (http://www.adea.org)
When the Committee on Admissions has examined your academic credentials, recommendations
and test scores and has made its decision on your application, you will be notified accordingly by this
office. Should you have any questions concerning your application or the necessary documents that
accompany it, please do not hesitate to contact the Creighton Dental Admissions Office where your

INSTRUCTION FOR MAKING APPLICATION
SUPPLEMENTARY APPLICATION FORM
Complete both pages of the supplemental application form and return it to the Creighton Dental
Admissions office. It should be accompanied by your photograph as indicated on the form. The
photo should be a passport-type photograph and can be taped or glued to the supplemental application.
OFFICIAL TRANSCRIPT(S)
You will need to send one official transcript from each institution attended to AADSAS. They will
calculate your GPA and send us those results and a list of your classes as part of your application.
RECOMMENDATIONS
We require either a letter from the Recommending Committee at your college OR two letters of
recommendation from college science instructors (preferably one each from biology and chemistry) and
one letter of recommendation from a college non-science instructor. If you have not had a non-
science course, you can substitute a letter from a dentist or employer. AADSAS will send a copy
of your recommendations to us. Check with the Admissions Office to ensure that your
recommendations have been received. They are necessary for your file to be complete and to be
reviewed by the Admissions Committee.
DENTAL ADMISSIONS TEST (DAT) SCORES
You are required to take the Dental Admissions Test (DAT) and have your scores sent
to AADSAS. They will send us a copy of the DAT scores with your main application. If your DAT
scores have not been forwarded, please contact the Department of Testing Services, American

APPLICATION SERVICE FEE
At the time an applicant submits an application through AADSAS, a $60.00 non-
refundable application service fee should be sent directly to the Creighton Dental Admissions Office.
This fee can be paid by a check or money order made payable to "CREIGHTON UNIVERSITY". You
can submit this fee with the completed supplemental application and photograph.
NOTE:
All credentials become property of Creighton University upon submission and are not returnable.
